The Deadline for Scholarships is March 5, 2009

The Kentucky Coalition of Nurse Practitioners and Nurse Midwives is pleased to announce our exciting new scholarship awards program for 2009. In addition to the two awards for MSN students, we now have one award for an ARNP or CNM pursuing a doctoral degree. Also, we are privileged to participate in the awarding of the Jacqueline Bail Nurse Practitioner Memorial Nurse Practitioner scholarship.

Each of the scholarships available are for $1,500.00. Scholarships will be awarded at our Annual Conference in Lexington on April 16, 2009.

Students interested in applying should review the eligibility and submission rules and send an entry in a 9x13 postage-paid envelope to the address listed below. Completed applications will be judged using “blind” review processing and a predetermined scoring system. Individual members of the scholarship committee from all over the state will independently judge each entry. Criteria for the judging includes academic performance, professional involvement, financial need, a letter of reference, a short essay, and a written statement of personal and professional goals.

There is no standard application form to complete. However, detailed instructions are provided and all items listed must be received in a single mailing to be considered for judging. Entries will be received until 6:00 p.m. on March 5, 2009. No late entries will be accepted; therefore, students are encouraged to have items postmarked with proper postage (first class mail), no later than March 1, to ensure delivery by the March 5, 2009 deadline. All entries become the property of the KCNPNM and will not be returned.

Winners will be notified within a few days of the Coalition Conference dates and will be invited to attend the Conference banquet on April 16, 2009. Winners will be announced to the public and awards distributed at that time. The winners’ names will also appear in the summer issue of KCNPNM News. The application process is not difficult and should not take long to complete. All MSN-ARNP and doctoral-ARNP students meeting the minimum criteria are encouraged to apply.

 Scholarship Rules and Eligibility


To be eligible:
1. Must currently be enrolled in an accredited ARNP program in the state of Kentucky. Doctoral students must be graduates of same, currently enrolled in a doctoral program.
2. Must have a minimum of 3.0 on a 4.0 grading scale for all graduate work or last 3 academic semesters if no graduate work has been completed.
3. Must have a minimum of 1 semester remaining prior to graduation from ARNP preparatory or doctoral program.
4. Must agree, upon notification of winning, to be responsible for any taxes assessed on the award by state, local, or federal agencies.


How to submit an entry for judging:
1. Packet must be submitted in a first class, postage-paid 9x13 envelope.
2. Packets must be received no later than 6 p.m. on March 5, 2009.
3. Packets become the property of the KCNPNM and will NOT be returned to the applicant.


How entries will be judged:
1. There are 6 categories for judging: academics, professional involvement, financial need, recommendation, personal and professional goals, and short essay.
2. All entries will be received by the committee chairperson, blinded and then distributed to the individual judges. The committee chair will not participate in the judging.
3. There are 31 points possible during the judging process. In the event of a tie involving more than two entries with the highest score, an alternate judge will perform the tie-breaking review. The entry with the highest score after that review will be declared the winner. There are no consolation awards.
4. The ruling by the judges is FINAL. Contesting the final judgment will not be permitted.


Rules:
ALL OF THE FOLLOWING RULES MUST BE FOLLOWED WHEN COMPLETING YOUR ENTRY. FAILURES TO COMPLETE THE PACKET OR COMPLY WITH THE RULES OF THE COMPETITION WILL RESULT IN THE ENTRY BEING WITHELD FROM JUDGING.

1. All pages are to be typed and photocopied (as applicable) on white paper with black ink in a standard text, size 10-12. No bold, italic, underlining, or colored fonts are to be used. No pages are to be folded.

2. The first page of the packet should contain a signed and dated cover letter. The statement “I (insert your name) am submitting this packet as my entry in the scholarship award program, after reading and understanding the official rules.” The cover should also include your name, address, daytime and evening phone numbers, email address (if available), school/program of study, enrollment date, and anticipated date of graduation. Please submit only 1 copy of 1st page.

2. The second page of the packet should be a list of any/all professional activities, including years of memberships in professional organizations, and explanations of your participation. Please submit 8 copies of 2nd page.

3. The third page of the packet will be divided into two parts. The first half of the page will be your personal attestation of financial need, and should include the total percentage of your self-funded school-related expenses. This calculation should be based on tuition, fees, books, and required materials NOT covered by scholarships, grants, or other sources of assistance. You may explain any extenuating or extraordinary circumstances you would like to have considered as needed, within the 1/2 page limit. The second half of the third page should be your statement of personal and/or professional goals.

Doctoral students should include information on research intentions, and how their opportunities as a doctorally prepared ARNP will benefit society. Please submit 8 copies of 3rd page.

4. The fourth page of the packet will be an essay, 1 page (max.) in length, titled “The Benefits of Belonging”. Describe how professional relationships are forged, partnerships built and the profession is advanced by being an active member in a state NP organization. Please submit 8 copies of 4th page.

5. The fifth page in the packet should be a cover page (1 copy) for the letter of recommendation (6th page) written for you by an ARNP, faculty member, or professional colleague. The only identifying information on the recommendation itself should be your student ID number. The cover page and the recommendation must be dated and signed by the writer, and is not to be written on letterhead. Please submit 8 copies of the 6thpage. Members of the KCNPNM Board of Directors, and Scholarship Committee are prohibited from writing recommendations for these awards.

6. The seventh and subsequent pages in the packet should be a single copy of your graduate transcripts. Undergraduate transcripts or your 3 most recent academic semesters may be submitted if no graduate work has been completed. Any unofficial transcripts must be signed and dated by your program administrator or faculty advisor.

You may include a self-addressed, stamped postcard in your packet if you desire a return receipt.
