Evidence suggests hospital readmissions and all-cause mortality rates are reduced when patients who have experienced a heart attack participate in a cardiac rehabilitation program. However, even as providers are striving to improve and sustain the quality of delivered cardiovascular care, this is often a missed opportunity.

During this webinar presented by the Home Health Quality Improvement National Campaign (HHQI), leading national experts will discuss the diseases/diagnoses that qualify a patient to participate in cardiac rehab and the services that are included. We’ll also review available alternatives for those patients who are unable to attend an outpatient program.

Learning Objectives: 

  1. Describe two of the Million Hearts 2022® cardiac rehabilitation priorities.
  2. Explain three benefits and potential barriers of outpatient cardiac rehabilitation.
  3. Discuss two alternatives for the home health setting when outpatient cardiac rehab is not possible.
  4. List three methods to improve home health patients’ cardiac rehabilitation participation.

Free Continuing Education for Nurses & Physical Therapists

This activity is pending approval of 1.75 hours of continuing education (CE) credit from the Alabama State Nurses Association, an accredited approver of nursing continuing education through the American Nurses Credentialing Center’s Commission on Accreditation (ANCC). An application is also pending for continuing competency units (CCUs) through the Federation of State Board of Physical Therapy (FSBPT). Visit the FSBPT website to see which states currently accept FSBPT certificates.

To receive CE credit, participants must watch the webinar and enroll in the corresponding course in HHQI University's Cardiovascular Health course catalog to complete related coursework. Step-by-step instructionswill be discussed at the end of the webinar.
